Print blank forms, fill them out by hand, and bring them to your appointment.
If you are unable to print the forms, please plan to come fifteen minutes early to your appointment to complete the forms in-office.
Completed forms may be faxed to 317-872-9303 or printed and delivered in person.

WHAT TO EXPECT AT THE FIRST VISIT
NEW PATIENT PAPERWORK
Above is a link to the new patient paperwork. Click and print off the paperwork and bring it in with you to your first visit. This will help expedite your first visit, which usually takes about an hour and a half. If you are unable to print the forms, please plan to come fifteen minutes early to your appointment to complete the forms in-office.
CONSULTATION
During your consultation, the doctor will discuss your needs in greater detail and will share with you what our clinic has to offer. You will then receive a comprehensive examination.
No unnecessary tests will be performed. All tests we administer are necessary for the complete understanding of what is involved with your case and will assist your doctor in prescribing a complete wellness program.
EXAMINATION
One of our skilled doctors will perform a detailed exam including the following painless procedures: three neurological scans; reflex and range-of-motion testing; standard orthopedic, neurological, and postural tests; and palpation of any painful areas to determine what tissues are affected and to what extent. Lastly, a pH saliva test will be performed to determine the alkalinity of your body. To ensure the accuracy of your pH test, please do not eat or drink anything except water for two hours prior to your appointment time.
X-RAYS
Your evaluation may require radiographs to rule out certain conditions, to better evaluate the structures, and to help develop the most effective treatment plan. If you have had X-rays taken within the last year prior to visiting our office, we ask you to bring them with you if possible.
REport OF FINDINGS
After your exam, your next visit will be scheduled as soon as possible. At this appointment, your consulting doctor will review your X-ray and exam findings, outline your recommended treatment plan and procedures, and give you home care and nutritional instructions. You may ask any questions you have at any time!
TREATMENT
Treatment in our clinic consists of B.E.S.T., structural alignment, and/or physical therapy, according to your individual treatment plan. The first treatment is usually performed on your second visit, following your Report of Findings.
